Membership

We are accepting new members at this time. After you fill out the enrollment form, we will schedule a one time, free visit for an introduction and overview of our practice. You can decide afterwards if you would like to enroll! Due to unclear federal regulations, we cannot serve Medicaid or Medicare patients at this time. We recommend all our members have hazard insurance to cover emergencies, hospitalizations & surgeries.
